1.1 PROJECT INFORMATION
A. Notice to Bidders: Qualified bidders may submit bids for project as described in this
Document. Submit bids according to the Instructions to Bidders.
B. Project Identification: Mosaic Health; Rushville Health Center Renovations
1. Project Location: 2 Rubin Dr., Rushville, New York, 14544, United States.
C. Owner: Mosaic Health.
1. Owner’s Representative: Denise Burgen, Mosaic Health.
D. Architect: HBT Architects, LLP.
E. Project Description: Project consists of Renovations and Alterations to the existing single story
(with basement) medical and dental health center. Renovations include finish and millwork
upgrades, enclosure of existing exterior stair and reconfiguration of several office and medical
service support spaces.
1. Project cost range is anticipated to be under $1,550,000.00.
F. Construction Contract: Bids will be received for the following Work:
1. General Contract (all trades).
1.2 BID SUBMITTAL AND OPENING
A. Owner will receive sealed lump sum bids prepared in compliance with the Instructions to
Bidders until the bid time and date at the location given below.
1. Bid Date: Thursday, November 17, 2022.
2. Bid Time: 3:00 p.m., local time.
3. Location: Office of HBT Architects, 2 Elton St., Rochester, NY 14607.
B. Bids will be thereafter privately opened.
1.3 BID SECURITY
A. Bid security shall be submitted with each bid in the amount of 5 percent of the bid amount. No
bids may be withdrawn for a period of 90 days after opening of bids. Owner reserves the right
to reject any and all bids and to waive informalities and irregularities.
1.4 PREBID MEETING
A. Prebid Meeting: A Prebid meeting for all bidders will be held at Rushville Health Center, 2
Rubin Dr., Rushville, NY 14544 on November 2, 2022 at 4:00 p.m., local time. Prospective
prime bidders are requested to attend.
1. Bidders’ Questions: Architect will provide responses at Prebid conference to bidders’
questions received up to three business days prior to conference.
1.5 DOCUMENTS
A. Printed Procurement and Contracting Documents: Obtain after 3:00 p.m. on October 24, 2022,
by contacting Rotolite-Elliott Corporation, One Grove St. – P.O. Box 97, Pittsford, NY 14534,
(585) 385-1463. Documents will be provided to prime bidders only; only complete sets of
documents will be issued.
1. Deposit: $100.00 made payable to the Owner. (Credit card payments will not be
accepted.)
2. Shipping: Additional non-refundable shipping charges of $50.00 will apply.
3. Any bidder returning sets promptly (within 30 days after date set for bid opening) and in
good condition, will be refunded the full deposit. A UPS or FedEx account number can
be substituted for the charge for mailing. Checks should be mailed to Rotolite – Elliott,
PO Box 97, Pittsford, NY 14534 payable to Mosaic Health.
4. All bid document requests can be completed through the www.recplanroom.com
in advance for pickup and or shipping from Rotolite-Elliott Corporation. (Visit the
www.recplanroom.com and click on “All Public Jobs”, under the Menu Tab on the lefthand
side of the plan room home page, select “Rushville Health Center”, add to cart and
proceed to check out.)
B. Online Procurement and Contracting Documents: Obtain access after 3:00 p.m. on October 24,
2022, by contacting Rotolite-Elliott Corporation; www.recplanroom.com (click on “All Public
Jobs” under the Menu Tab on left-hand side of the plan room homepage. Once you are in the
public plan room, please select the link for “Rushville Health Center”.) Online access will be
provided to all registered bidders and suppliers.
1. The cost to Download files is $59.00 and payable through the plan room with credit card
only.
2. Upon download, said entity will be added as a legitimate plan holder on the job and will
receive electronic files of any bid addendums issued throughout the bidding process.
1.6 TIME OF COMPLETION
A. Successful bidder shall begin the Work on receipt of the Notice to Proceed and shall complete
the Work within the Contract Time.
1.7 BIDDER’S QUALIFICATIONS
A. Bidders must be properly licensed under the laws governing their respective trades and be able
to obtain insurance and bonds required for the Work. A Performance Bond, separate Labor and
Material Payment Bond, and Insurance in a form acceptable to Owner will be required of the
successful Bidder.
B. Bidders must be able to demonstrate experience and successful completion of a minimum of
Three (3) projects of similar size, scope and complexity in compliance with Article 28, State
Hospital Code/Public Health Law.
C. Bidders related to, or financially interested in, Mosaic Health are not eligible to enter into a
Contract with Mosaic Health.
D. Bidders restricted by Executive Orders 12549 and 12689 “Debarment and Suspension” are not
eligible to enter into a Contract with Mosaic Health.
E. Participation by Minority-Owned, Women-Owned or Service Disabled Veteran-Owned
business enterprises is encouraged.
1.8 NOTIFICATION
